In reply As an avid supporter and practitioner of controlled clinical trials, I find it hard to disagree with the approach advocated by Salinas and Garner. The view that I advanced in my commentary was based on my understanding of the literature at the time. If the analysis being carried out by the Parasitic Diseases Group in the Cochrane Collaboration shows no unequivocal benefit for any one drug, then I would consider a placebo-controlled trial not only ethical but also imperative.A senior British neurologist once chided me by suggesting that my word processor contained a sentence asking for a controlled clinical trial in every commentary that I wrote for the "Controversy" section. This is a rare example of British overstatement, but I do believe that one scientifically demonstrated fact is worth a hundred opinions.
