In Reply.  —Nadeau contributes a fresh viewpoint to decision making regarding levodopa therapy. Although the concept of "risk aversion" is relevant and quantifiable, it does not take due account of the influence that the physician and others may have on the patient's perception of risk, nor of the fact that "risk aversion" may be conditioned by depression and cognitive impairment, two common accompaniments of Parkinson's disease. The claim that risk aversion, or, indeed, any aspect of clinical decision analysis1 can definitely settle controversies is, to say the least, controversial.
